Hamilton Health Department Death Records

Hamilton is one of the few Ohio cities with its own health department that handles death certificates. The City of Hamilton Health Department maintains vital records for deaths that took place within the city limits. This gives Hamilton residents a local option instead of having to go through the county. The city health department can issue certified copies for deaths in Hamilton going back to 1908.

The process is straightforward. Visit the health department during business hours with the name of the deceased and the date of death. Staff can pull the record and issue copies while you wait. The fee is about $25 per certified copy. You can also request by mail. Send a letter with the full name of the deceased, date of death, your name and relationship, a return address, and payment by check or money order. Include a self-addressed stamped envelope for the return.

The county health department may have records that the city office does not, and the reverse can be true as well. If one office cannot find a record, try the other.
