Cincinnati Health Department Death Records
Cincinnati is one of the few Ohio cities with its own health department that handles vital records. The Cincinnati Health Department issues death certificates for deaths that took place within Cincinnati city limits. This is different from most Ohio cities, where the county handles everything. If someone died inside Cincinnati, this is your first stop.
The department can issue certified copies for deaths from 1908 to the present. Walk-in requests during business hours are the fastest way to get copies. You can also submit a written request by mail. Include the deceased person's full name, date of death, your name and address, and payment. The Cincinnati Health Department is located in the city government complex. Call ahead to check current fees and hours.

For deaths that happened outside Cincinnati but still in Hamilton County, you need to go through Hamilton County Public Health instead. The two offices cover different areas. Make sure you know where the death occurred before you request a certificate.
Hamilton County Death Index
Hamilton County Public Health handles death records for the parts of Hamilton County outside Cincinnati. If you are not sure whether a death happened inside or outside city limits, try both offices. The county also sends all death records to the Ohio Department of Health in Columbus. The state can issue certified copies for any Ohio death from 1908 forward. Under Ohio Revised Code Title 37, death certificates are public records and anyone can request a copy.
For older records, the Hamilton County Probate Court holds death records from the 1800s. Cincinnati is one of Ohio's oldest major cities, so the historical death record collection is large. The probate court has ledger books with handwritten entries showing name, date, age, cause, and sometimes birthplace. These pre-1908 records are the only official source for deaths before the state system started.

Cincinnati straddles the Ohio-Kentucky border. If you cannot find a death record under Hamilton County or the city, consider that the person may have died across the river in Kentucky. Northern Kentucky cities like Covington and Newport are just minutes from downtown Cincinnati. Many people who lived in the Cincinnati area died in Kentucky hospitals or nursing homes. Check Kentucky records if your Ohio search comes up empty.
Note: Deaths in Kentucky are filed with the Kentucky Office of Vital Statistics, not Ohio, even if the person lived in Cincinnati.

The Ohio Death Certificate Index is free. It covers 1913 to 1944 and 1954 to 1963. Search by name and look for Hamilton County results. The index provides the certificate number you need to order the actual document. Cincinnati deaths from those years will show up under Hamilton County in most cases.
